If you, or someone you know, needs assistance obtaining funding for any medical equipment, braces, gait trainers, therapy tricycles, shower chairs, etc. that is not covered by insurance and are 21 years of age or younger, email or call us and request an application.

Please follow the instructions on the application and complete it in its entirety, mail it to us and we will review your financial need.

Application process:

1) Complete Application for Assistance and return via mail to the address below. No faxes please.

2) Allow 3-4 weeks for review of your application.

3) Applicant may be subject to an interview.

4) Should this application be accepted, funds will be distributed to the company where equipment is being purchased, no funds will be directly released to applicant.

Disclosure:

Wheel to Walk Foundation reserves the right to accept or decline any applicant. The Wheel to Walk Foundation will not discriminate based on race or religious background. The acting directors of the Wheel to Walk Foundation will decide on which applicant will receive funding. If accepted, no applicant will receive funds directly from Wheel to Walk, all funds will be distributed to location of equipment or service.
